Once you have all the above forms completed, please come into the Police Station for application review and processing.
You will need two forms of identification:
One form of ID must be your Driver’s License with a Long Branch street address (no PO Box addresses)
Additional forms of ID: Firearms Identification Card (must be presented if applicant has an FID card), Social Security card, Birth Certificate, Passport, Vehicle Registration, Auto Insurance Card, Voters Registration Card, Utility Bill (must be within previous 60 days of application date), Bank Statement (must be with previous 30 days of application date), City/State ID, and/or Green Card
If you have never possessed a FID Card, you will be required to be fingerprinted and will be given instructions on where to report.
If you possess a FID card, please bring that with you as well as your driver’s license.
Once the initial application is accepted you will receive instructions on obtaining finger prints (in order for application to be approved fingerprinting must be completed within 30 days of application.

Once on the website click on the ON LINE FORM 212A, (a highlighted block located in the lower left side of the page).
Follow the prompts for demographic and payment information. Payment can be made with a credit or debit card.
Upon completion of the form you will receive an email confirmation & receipt that will include a confirmation number. IMPORTANT: Print out the confirmation page for your records and return a copy to us by either:
